How to Pay for In-Home Care

How to Pay for Home Care in Nebraska

Jump to a state to see its Medicaid programs, waivers, and other ways to help pay for in-home care.

This directory lists Nebraska-specific and federal payment sources that can pay for, reimburse, offset, or help plan for home care. Some are direct payment sources; others are indirect supports that reduce the amount of private home care a family must buy. Nebraska's strongest state-specific pathways include the Aged and Disabled Waiver, Personal Assistance Services, Disabled Persons & Family Support, Lifespan Respite, the Nebraska Caregiver Tax Credit, and PACE/Immanuel Pathways, and VA benefits are broken out because they can be one of the strongest payment pathways for eligible Nebraska veterans and surviving spouses.

Before You Rely on Any Payment Source

  • Screen AD Waiver and PAS separately

Nebraska has both the Aged and Disabled Waiver and state-plan Personal Assistance Services. Ask DHHS which pathway fits the person’s level of care and service needs.

  • Ask about paid family caregiving

PAS can pay certain family members who are not legally responsible for the participant. The AD Waiver also has LRI Personal Care for certain situations. Get written approval before assuming a relative can be paid.

  • Use DPFS when Medicaid is not the only answer

Disabled Persons and Family Support can authorize up to $400 per month / $4,800 annually for qualifying disability-related services, including personal care and housekeeping.

  • Do not miss Nebraska Lifespan Respite

Eligible caregivers may receive up to $125/month for planned respite, plus a separate exceptional/crisis respite request up to $2,000 per eligibility period.

  • Nebraska has a caregiver tax credit

The state credit equals 50% of qualifying caregiver expenses, up to $2,000, or $3,000 if the eligible family member is a veteran or has dementia. It is nonrefundable and subject to income/program caps.

  • PACE is geographically limited

Immanuel Pathways is currently Nebraska’s only approved PACE provider and does not serve the entire state. Confirm the exact address/ZIP.

  • Use ADRC as the navigation hub

The Nebraska ADRC can connect families to AAAs, in-home personal care, homemaking, transportation, meals, benefits assistance and long-term-care options counseling.

  • Do not confuse Medicare with long-term custodial care

Medicare may cover qualifying skilled home health and limited aide services, but not ongoing personal care when that is the only need.

  • GUIDE is not a cash allowance

Medicare GUIDE provides dementia-care coordination and qualifying respite through participating organizations, not unrestricted cash paid to families.

  • VA CCN requires authorization

Nebraska is VA CCN Region 2, administered by Optum Serve. Obtain VA referral/authorization before routine community care.

  • Use property-tax relief to preserve care dollars

Nebraska homestead exemptions may help eligible age-65+, disabled and veteran households reduce property taxes and redirect cash toward care.

  • Document private family payments

If a relative is paid privately, use a prospective written care agreement, timesheets and proper payroll/tax treatment—especially if Medicaid may be needed later.

Program rules, income limits, and availability change over time and can vary by county. Confirm current details with the program directly, or ask a home care provider in Nebraska to help you navigate the application process.
