Advocate Care Management

Advocate Care Management is a care management organization serving Indianapolis and surrounding areas since 2010. Its team of registered nurses and a medical social worker assesses each client's needs, writes a customized plan of care, arranges services, monitors that care and keeps everyone involved informed. The care manager coordinates and supervises all care, skilled or non-skilled, whether the client lives in their own home, a family member's home or an assisted living community. Services include advocacy, assessment, crisis intervention, education, guidance for adults with disabilities, information and referral, care management for Alzheimer's and dementia, and free assisted living placement.
