Concierge home care is not a different set of tasks. The bathing, the meals, the driving, and the companionship are the same as any personal care arrangement. What is being bought is management and consistency: a smaller caseload, caregivers selected rather than assigned, and someone whose job is to notice when the plan no longer fits.
Care management is the substantive part of it, and it is a real discipline that also exists on its own, sold as geriatric care management or aging life care. A care manager assesses the situation, writes the plan, revises it when the condition changes, attends appointments, and keeps the physicians, the therapists, and the family working from the same information. Families who have tried to do this themselves across a hospital stay, a rehab stay, and a discharge usually recognise the value immediately.
It is almost always private pay. Long-term care insurance will often cover the hands-on caregiving hours while excluding the management layer, so the two get billed differently, and Medicare does not cover either. That makes the rate structure the thing to read closely rather than skim.

Worth asking before signing: who specifically is the care manager and what are their credentials, how often do they reassess, are the same caregivers guaranteed or is continuity a best effort, what happens on a call at two in the morning, and which of these things are inside the hourly rate against billed separately.
